Keirse is professor of Obstetrics at the University of Leiden, Netherlands. james Neilson is Professor of Obstetrics and Gynaecology at Liverpool University. Mary Renfre is a Midwife Researcher at the National Perinatal Epidemiology Unit, the Radcliffe Hospital, Oxford.
