The 4th ed. was cited in Brandon/Hill. The 5th ed. has been substantially revised and reorganized, with added emphasis on surgical techniques. The CD-ROM contains bibliographies for each chapter; the chapters contain a shorter list of the most crucial references. The chapters are grouped into sections on the topics of basic principles, hand, wrist, elbow and forearm, nerve, congenital disorders, bone and soft tissue reconstruction, and other disorders. Individual chapter topics of the final section include amputations, compartment syndrome, factitious disorders, and the management of snake (pit viper) bites. Green and co-author William C. Pederson are with the department of orthopaedics of the U. of Texas Health Science Center, San Antonio. The other two co-authors are Robert N. Hotchkiss of the Alberto Vilar Center for Research of the Hand and Upper Extremity, New York City, and Scott W. Wolfe of the Weill Medical College of Cornell U.
