Dietrich Doerner's The Logic of Failure is a fascinating read. He's talking about a serious subject - making complicated decisions in complex situations - and discusses it in a thoughtful, positive, and encouraging way. Some of his descriptions are irresistibly funny (as is the photo on the cover of the paperback edition), and others are grimly compelling - he gives a brief but thorough explanation of what exactly happened at Chernobyl, for instance. If you work in a complex system, you will benefit by reading this book.
